I was born in Indianapolis,
Indiana, and grew up in Dayton, Ohio. I could hardly wait to get out of
those villages and move to the big city (Chicago). I am of Dutch and
German descent.
My last name originally was "Heusinkveld", but my great-great-grandfather tired of the
mispronunciations and changed it. By the way, some of my pediatric
patients call me "Dr. Crazyfield", which I really do enjoy!

wife Barbara and his two sons Anthony and Andrew to enter
private practice in Otolaryngology - Head and Neck
